Here you'll find the most common forms used at Aspen Creek Academy.


Registration Form
This is the initial form you'll need for enrolling at Aspen Creek Academy. To hold your spot for your child’s attendance, all enrollment information must be completed, signed, and returned to the school’s office along with the nonrefundable registration/supplies fee of $85.

Child Checklist
This checklist is used to confirm that you have received and completed all necessary forms and evaluations for enrollment.

Child Pick-Up
At Aspen Creek Academy children must be picked up by 6:00 pm. This contractural form confirms that parents and guardians understand the child pickup proceedures.

Field Trip Release
Field trips are great learning fun! Make sure this form is completed before every trip.

Transportation Consent
This form authorizes walking trips and riding in Aspen Creek Academy vehicles.
Emergency
Medical Care Consent
This is Medical Release gives permission to Aspen Creek Academy to call a doctor for medical care for should an emergency arise.



Physician Health Form
For both parents and physicians to complete, this form identifies any health concerns and confirms that your child may participate in routine activities.

Medicine Administration
This is Medical Release form....

Sunscreen Permission
Permission form to apply sunscreen before going outside on warm sunny days.
Miscellaneous
Genreal release form for television viewing permission, use of sunscreen, and permission to photograph.
